Mr. P is a 76-year-old male with cardiomyopathy and congestive heart failure who has been hospitalized frequently to treat CHF symptoms. He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. He has no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him.


Considering Mr. P’s condition and circumstance, write an essay of 500-750 words that includes the following:

It has been noted, going through the case, that Mr. P, a 76-year-old male, is suffering from cardiomyopathy and congestive heart failure (CHF) and he has been hospitalized frequently to treat CHF symptoms. While managing his polypharmacy Mr. P finds it difficult to maintain his diet restriction. The medical condition of Mr. P has put his wife in utter distress. She is unable to get out from home due to taking care of her husband and she is overwhelmed with the stack of medical bills. Moreover, the health condition of Mr. P has made him despondent and he is willing to embrace death. 
Considering the psychological condition of the couple some counselling must be provided to both of them. Both the patient and his wife must be educated about the ailment. The care approach in this respect should be a poised one in which both the psychological and physical aspects of the patient and his wife must be considered. Mr. P’s wife should be made understood that there is a need of medication for Mr. P and that the medical bill incurred should be considered relevant and important as it is needed to save the valuable life of Mr. P. MR. P should be made understood the value of life and the scopes that he has to spend a quality life despite having the ailments. 

The counselling session would be arranged to make Mr. P and his wife understand the importance of medication. As a caregiver and counsellor, one should make Mrs. P understand why Mr. P would be in need of medication throughout his life. Through the counselling approach the treatment should be enriched in a thorough manner. It must be noted that through counselling, Mr. and Mrs. P should be made understood why it is important for Mr. P to be on medicine and to be in a continuous medication regime. Mrs P. should be conveyed the fact that it is important to incur the medical expenditure as medicines is needed to sustain the pumping ability of Mr. P’s heart (Mayo Clinic, 2018). The medicines are also meant for improving the blood flow, to lower the blood pressure, to slow the heart rate, to remove excess fluid from Mr. P’s body and to keep the blood clots from forming (Mayo Clinic, 2018). And due to all these reasons, medicines should be continued and taking medicines in a timely manner would make it possible for Mr. P. to lead a quality life in the long-run. Moreover, it is by citing the fact that medicines are needed to sustain a quality life for Mr. P that Mrs. P should be made understood the relevance of the medical bill in this case. 
Moreover, as a part of the teaching plan for treatment, Mrs. P should be taught to refrain from providing any stress to Mr. P and she should be taught to be more collaborative in this respect. Also, Mrs. P. should be counselled in order to empower her with the decision making capacity that is needed to control Mr. P’s low salt diet (Mayo Clinic, 2018).
